Chronic Disease Self-Management Program

The Chronic Disease Self-Management Program (CDSMP) is a 6-week workshop that meets for 2 ½ hours per week and helps older adults self-manage their chronic conditions.

Research has shown that individuals with various chronic conditions have similar concerns and problems.  The goal of these workshops is to provide support and tools to help individuals deal not only with their disease(s) but also with the impact these diseases have on their lives and emotions.

A list of ongoing and upcoming programs can be found below.  Interested individuals can register by calling the contact information for the session they are interested in.  You may register for a program already in process.

For general information or questions about the Chronic Disease Self-Management workshops contact your local Area Agency on Aging.
